World experience in the use of assisted reproductive technologies in recipients of solid organs

The article analyzes current literature data on the possibility of using assisted reproductive technologies (ART) in solid organ recipients. The following aspects are considered in the paper: the key factors of infertility pathogenesis, the effect of immunosuppressive therapy on fertility, the timing of restoration of reproductive function and the possibility of using ART. Organ dysfunction and immunosuppressive therapy have been shown to significantly reduce fertility, but the use of modern reproductive technologies makes it possible to achieve a successful pregnancy. The key factors are the timing of conception planning (not earlier than 1–2 years after transplantation), safe correction of immunosuppressive therapy and individual selection of stimulation protocols.
Conclusion. The problem of achieving conception after solid organ transplantation requires an interdisciplinary approach involving transplant surgeons, obstetricians, gynecologists, and reproductive medicine specialists. Despite the success of ART, the safety of immunosuppressive therapy during pregnancy remains a challenge. Optimization of ovarian stimulation protocols taking into account the type of transplant and any concomitant therapies as well as the development of unified recommendations for managing pregnancy in recipients are also important issues that need to be addressed. Further research in this area will improve pregnancy outcomes and expand childbearing opportunities for women after transplantation.
