Implementation of clinical guidelines "Preeclampsia. Eclampsia. Edema, proteinuria and hypertensive disorders during pregnancy, childbirth and postpartum period" in medical organizations of the regions of the Russian Federation

Objective. To evaluate the implementation of the clinical guidelines ‘Preeclampsia. Eclampsia. Edema, proteinuria and hypertensive disorders during pregnancy, childbirth and postpartum period’ and the existence of the practice of determining preeclampsia markers (sFlt/PlGF) in medical organizations of the regions of the Russian Federation within the framework of the organizational and methodological guidance of the National Medical Research Center for Obstetrics and Gynecology.
Materials and methods. This was an analysis of 83 analytical reports on the results of field visits aimed at the examination and analysis of the organization of healthcare in the field of ‘obstetrics and gynecology’ for the period 2022-2025 in 66 regions of the Russian Federation. As part of the field events, the specialists of the National Medical Research Center assessed the level of implementation of the clinical guidelines ‘Preeclampsia. Eclampsia. Edema, proteinuria and hypertensive disorders during pregnancy, childbirth and postpartum period’ and the practice of determining preeclampsia markers (sFlt/PlGF) based on primary medical records (completed cases of medical care from 2021 to 2024, n=419) in medical organizations of the regions of the Russian Federation.
Results. The analysis showed that 100% of the criteria for the quality assessment for the clinical guidelines ‘Preeclampsia. Eclampsia. Edema, proteinuria and hypertensive disorders during pregnancy, childbirth and postpartum period’ were met in medical organizations in 27 (40.9%) regions of the Russian Federation and that there was practice of determining preeclampsia markers (sFlt/PlGF) in medical organizations in 20 (30.3%) regions of the Russian Federation.
Conclusion. The assessment of the implementation of clinical guidelines ‘Preeclampsia. Eclampsia. Edema, proteinuria and hypertensive disorders during pregnancy, childbirth and postpartum period’ and the existence of the practice of determining markers of preeclampsia (sFlt/PlGF) in medical organizations of the regions of the Russian Federation requires optimization of the organizational and methodological work by both healthcare organizations and regional executive authorities aimed at improving the quality of medical care.
