Synchronous endometrial cancer of uterine hemi-cavities in uterus didelphys

Rationale. Endometrial cancer associated with uterine anomalies is extremely rare and is characterized by high diagnostic complexity. A comprehensive assessment of uterine cavities is crucial for gynecological practice, as it identifies the extent of the lesion and the appropriate treatment strategy. 
Case report. A case study describes of a 68-year-old female patient with complete duplication of the uterus and incomplete duplication of the vagina, who was diagnosed with synchronous endometrial cancer of uterine hemi-cavities (FIGO IB and IIIA). Preoperative imaging (MRI, ultrasound) made it possible to diagnose a tumor only in the right hemi-uterus, while involvement of the left hemi-cavity was only confirmed by postoperative morphological examination. A total hysterectomy of both uterine hemi-cavities and adnexa was performed followed by adjuvant radiation therapy.
Conclusion. The obtained results highlight the need for separate diagnostic curettage of uterine hemi-cavities in cases of complete uterine duplication and demonstrates the feasibility of using a comprehensive preoperative examination algorithm (3D ultrasound, MRI, hysteroscopy) to optimize oncological outcomes.
