Asymptomatic uterine scar rupture: diagnostic value of ultrasound scan in an early postpartum period

Rationale. Given the increasing frequency of cesarean sections, uterine scar rupture remains a life-threatening obstetric complication. Its asymptomatic nature makes it particularly challenging to diagnose.
Case report. A rare case of asymptomatic complete uterine scar rupture is described in this article. The pregnancy was uneventful. A successful vaginal delivery was performed at 40 weeks of gestation. The patient's condition remained satisfactory after delivery, with no complaints or clinical symptoms of rupture. However, scar rupture with the formation of a hematoma was detected only during a routine ultrasound scan performed 30 minutes after delivery due to the obstetric history of the puerpera. An emergency laparotomy with organ-sparing metroplasty was carried out. The patient was discharged in satisfactory condition on the 4th day.
Conclusion. This case illustrates the possibility of asymptomatic uterine scar rupture after successful vaginal delivery. The classic clinical picture may be absent, which increases the risk of late diagnosis and the development of fatal complications. This case marks the high diagnostic value and potential need for routine use of ultrasound scan in the early postpartum period in patients with an uterine scar for the timely detection of such complication.
