Clinical portrait of female patients and causes of early pregnancy failure in the Tyumen region

Objective. To assess the prevalence of early pregnancy failure (EPF) in the Tyumen region and to identify the most significant social and clinical features of women with EPF.
Materials and methods. We used statistical data from the Ministry of Health of the Tyumen Region, covering both the period of 2021–2025 and visits to the Tyumen Perinatal Center. The analytical data enrolled 1,457 patients with EPF. Examination methods covered history data, gynecological examination, pelvic ultrasound, and histological examination of gestational tissue.
Results. The prevalence of spontaneous miscarriages (SM) among all pregnant women in the Tyumen region averaged 14.0% over a 5-year period. The percentage of SM within EPF was 49.0%, with the incidence of SM increasing by 5.2% and EPF by 2.0% from 2021 to 2025. Anembryonic type EPF was observed in an average of 12.0% of patients, and embryonic death type EPF – in 88.0%. The percentage of SM among the total number of patients registered at the Tyumen Perinatal Center was 54.3%, while the EPF proportion within SM was 97.6%. EPF of the anembryonic type was observed in 13.8% of patients, with the embryonic death type – in 86.2%. Over a five-year period, the clinical profile of patients with EPF observed at the Tyumen Perinatal Center underwent changes, characterized by a 1.5-fold increase in the patients aged over 41 and an increase in genetically specified losses from 11.8 to 15.1% (p<0.001). A statistically significant increase in the number of unsuccessful ART cycles was revealed in EPF patients (from 1 to 4 in the upper quartile Q3; p=0.003), indicating a concentration of women with severe forms of reproductive dysfunction. A high prevalence of chronic endometritis (80.2%) was noted due to an increase in risky sexual behavior associated with social factors, and a shift in the period of ultrasound EPF finding from 6 to 7 weeks of gestation.
Conclusion. Following stable incidence of SM over the period of 2021–2025 (approximately 14%), an increase in SM and EPF with embryonic death being the predominant cause was recorded in the Tyumen Region. The clinical profile of patients diagnosed with EPF at the Tyumen Perinatal Center is characterized by an increased number of women of late reproductive age with a history of unsuccessful ART attempts, an increased number of genetic abnormalities, a high incidence of chronic endometritis, and an upward shift in the median ultrasound diagnosis of EPF by one week.
