Stress-induced cardiomyopathy (takotsubo syndrome) complicated by cardiogenic shock in a pregnancy with cesarean delivery

Relevance: Stress-induced cardiomyopathy (takotsubo syndrome) is a rare type of nonischemic cardiomyopathy characterized by acute onset and relatively rapid reversal development of left ventricular systolic dysfunction without atherosclerotic lesions of the coronary arteries. Early complications of this pathology include cardiac arrest, tachy- and bradyarrhythmia, acute heart failure, shock, apex thrombosis, stroke, rupture of the interventricular septum and free wall of the left ventricle. The mortality rate of patients with stress-induced cardiomyopathy (takotsubo syndrome) is as high as 5.6%.
Case report: The article presents a rare case of stress-induced cardiomyopathy (takotsubo syndrome) complicated by acute cardiac insufficiency and cardiogenic pulmonary edema, which developed during planned abdominal surgical delivery in a patient with preterm pregnancy; the situation required the participation of multidisciplinary specialists in emergency care (obstetrician-gynecologists, anesthetists, cardiologists, neonatologists, trained nursing staff), that contributed to the successful outcome of the presented case.
Conclusion: It is necessary to remember about the risk of stress-induced myocardial damage in patients of any profile, including pregnant women, especially if the patient has increased anxiety about the upcoming surgical intervention.
