Breast cancer remains the most common malignancy diagnosed in women of reproductive age, making fertility preservation and subsequent achievement of pregnancy clinically significant issues. This review presents persistent controversies concerning the effects of various polychemotherapy regimens on ovarian reserve and provides systematic overview of available fertility preservation strategies in breast cancer patients. Special attention is paid to modern stimulation protocols, including random start and DuoStim regimens, combined with aromatase inhibitors and antiestrogens in patients with hormone sensitive breast cancer. This paper reviews the conflicting evidence regarding the impact of germline mutations on ovarian reserve. Particular emphasis is placed on methods for maintaining reproductive function following the completion of chemotherapy, depending on whether ovarian function has been preserved or not.
Conclusion. Cryopreservation of oocytes and embryos remains the gold standard in oncofertility. Areas for further research include investigating the effect of new classes of anticancer drugs on ovarian reserve and pregnancy outcomes, assessing the long-term safety of discontinuing endocrine therapy in patients at high risk of recurrence, and validating genetic markers of premature ovarian failure to enable more accurate prediction of individual risk and the selection of the optimal fertility preservation strategy.
