New trends in breast cancer screening. The effect of epigenetic therapy on breast tissue condition in women with benign breast dysplasia
Yureneva S.V., Yakushevskaya O.V., Anisimkova A.A.
The importance of widespread breast cancer screening lies in the implementation of a two-stage strategy: the detection of background and pre-cancerous conditions, as well as the diagnosis of malignant neoplasms at the preclinical stage. Regular mammographic examination of women 40–75 years old with an interval of 1–2 years can reduce mortality from breast cancer by 25–35%. The assessment of breast cancer risk factors is fundamental for determining individual screening strategies and tailored management of women. Researchers and clinicians in the oncology community are primarily focused on developing and implementing strategies aimed at addressing modifiable risk factors for breast cancer, reducing incidence and mortality rates among cancer patients, and improving their survival rates. Each tumor has its own unique pathogenetic profile. The genome determines the key morphological characteristics and ‘behavioral’ features of tumors. However, despite the major importance of the ‘genetic basis’, epigenetic processes play an equally significant role in the development of neoplasms. In the context of breast disease, there is extensive discussion regarding the role of early epigenetic alterations in cells and, consequently, the potential for developing an oncoprotective approach in medicine.
This review presents the key strategies for secondary prevention of breast cancer based on mammography screening and the management of underlying benign breast conditions. The article provides a comparative assessment of national protocols for the management of patients, taking into account individual breast cancer risk, and outlines the efficacy profile and experience of using the medication indole-3-carbinol (Indinol Forto, ALCEA LLC, Russia), a compound known for its epigenetic mechanism of action. The search for relevant information was conducted in accordance with the stated topic using keywords to index articles in search engines and on the platforms PubMed, Google Scholar, ScienceDirect, CyberLeninka, and eLIBRARY.RU over the last 15 years.
Conclusion. Efforts to reduce the incidence of breast cancer must begin with the development of clear preventive medicine protocols, the essence of which lies in addressing modifiable risk factors for breast cancer (treating obesity, giving up harmful habits, optimizing physical activity, etc.), regular screening, early diagnosis and correction of morphofunctional changes in the breast that predispose to the development of a neoplastic process. Indinol Forto is registered as a non-hormonal medication with a proven effect on the structure of breast tissue. Preliminary data obtained during research into the efficacy/safety profile of the medication Indinol Forto®, which acts via an epigenetic mechanism, are opening up new horizons for preventive oncology.
