Incretin receptor agonists and PMOS: pathogenetic rationale for therapy

Polyendocrine metabolic ovarian syndrome (PMOS), which emphasizes the systemic nature and terminologically expands the etiopathogenesis of polycystic ovary syndrome (PCOS), is an important medical and social problem. Its significance stems from its impact on women’s well being and its negative demographic effects. Forecasts show no considerable rise in the global birth rate in the coming years. Since PMOS is linked to anovulatory infertility and miscarriage risk, it is crucial to enhance diagnostic approaches, deepen understanding of its pathogenesis, and refine treatment strategies. Overweight, obesity, insulin resistance, and related metabolic and cardiovascular disorders in women with PMOS substantially threaten their health and quality of life.
Current data on the etiopathogenesis of PMOS are systematized, with a focus on the multifactorial nature of the disease. Incretins have been studied: their role in PMOS pathogenesis and their metabolic and reproductive effects are described. The potential applications of incretin receptor agonists (GLP-1RAs, GLP-1RA/GIPRAs) in the treatment of PMOS are analyzed. Their effects on weight loss, correction of insulin resistance, lipid and carbohydrate metabolism, and impact on reproductive function are demonstrated. The limitations and specific features of incretin therapy are outlined, including issues related to contraception and pregnancy planning.
Conclusion. Despite the variety of available pharmacological approaches, the basic recommendation for correcting PMOS remains lifestyle modification. Bioinformatic methods help identify new therapeutic targets by analyzing disease mechanisms and promising intervention points. Large-scale research is required to develop reliable and scientifically supported recommendations.
