Therapy for genital mycoplasma infection: results of a prospective study

Rationale. In recent years, diseases caused by genital mycoplasmas have become a leading reason for urogenital pathology. However, antimicrobial resistance among these microorganisms is increasing, necessitating the use of drugs with proven efficiency.
Objective. To evaluate the clinical and laboratory efficiency of Vilpramycin SAR (josamycin) 1000 mg in the treatment of genital mycoplasma infection.
Materials and methods. 26 patients with genital mycoplasma infection caused by M. genitalium (n=8), M. hominis (n=8), and Ureaplasma spp. (n=10) were enrolled in the study. Research methods: microscopic, molecular-biological. Vilpramycin SAR was prescribed at a dose of 1000 mg twice a day for 10 days.
Results. Four weeks after the end of treatment, the absence of subjective and/or objective manifestations of the infectious and inflammatory processes caused by genital mycoplasmas were identified in 7/8 (87.5%; p=0.0327) patients of the 1st group, 7/8 (87.5%; p=0.0327) patients of the 2nd group and 9/10 (90.0%; p=0.0020) patients of the 3rd group. Eradication of M. genitalium was achieved in 100% of patients (p=0.0078), with eradication of M. hominis and Ureaplasma spp. in 77.8% (p=0.0020) of patients given a statistically significant decrease in the inflammatory response indices (95% CI for the difference in proportions 47.3–99.7; p=0.0327). Genital normocenosis was achieved in 8/8 (100%; p=0.0078) patients of the 1st group, 6/8 (75.0%; p=0.0327) patients of the 2nd group and 8/10 (80%; p=0.0020) patients of the 3rd group.
Conclusion. Vilpramycin SAR (josamycin) at a dose of 1000 mg 2 times a day for 10 days demonstrates high efficiency in the treatment of genital mycoplasma infection, and can be characterized by safety and tolerability, ensuring treatment compliance.
