Use of multicomponent biomimetic materials of extracellular matrix in regenerative medicine: from biology to clinical practice

Chronic dystrophic processes – genitourinary syndrome of menopause (GSM), stress urinary incontinence (SUI), lichen sclerosus, post-radiation and post-operative cicatricial skin changes – represent a significant medical and social problem leading to a persistent decrease in the quality of life. Such traditional treatment methods as local hormonal therapy or surgical interventions often affect symptomatic alterations and have a number of limitations and side effects. Implementation of the principles of regenerative medicine offers opportunities for pathogenetic treatment by activating the body's own reparative processes. In the modern paradigm, the key tool of this approach is multicomponent biomimetic materials of the extracellular matrix (ECM), reproducing its structure and function.
In this article we analyze the ECM line of multicomponent biomimetic materials produced in Russia (injectable biopolymer hydrogels) for the treatment of urogynecological pathologies. The composition, mechanisms of biomimetic action, and strategy for personalized modification selection depending on the clinical scenario are also discussed in detail. According to available clinical data, hydrogels are highly effective in restoring tissue trophism in GSM, as a regenerating agent for mild to moderate SUI, and in the treatment of sclerotic and cicatricial processes. Here we also touch upon safety issues, regulatory aspects, and prospects for technology development, including the creation of combined cell-engineered formations.
Conclusion: Multicomponent biomimetic hydrogels are a promising class of agents for minimally invasive, pathogenetically substantiated correction of a wide range of chronic degenerative diseases in urogynecology.
