Resolution of the XIX All-Russian Scientific and Educational Forum «Mother and Child» and the VI Congress of Obstetricians and Gynecologists of Russia

On September 26-28, 2018, the XIX All-Russian Scientific and Educational Forum “Mother and Child” and the VI Congress of Obstetricians and Gynecologists of Russia were held in Moscow.

According to the official statistics, the Forum and the Congress were attended by 4,394 doctors from 79 territorial entities of the Russian Federation, 344 cities and towns, as well as from 28 countries of near and far abroad.

The Congress summed up the work of the Russian Society of Obstetricians and Gynecologists (RSOG) over the past five years and discussed the tasks of the specialists aimed at the improvement of medical care availability and quality. The Congress also included the discussion on the contribution of obstetricians and gynecologists to the solution of demographic problems of the country in the next period.

As the participants of the Congress noted, there have been significant improvements in the system of maternal and child health protection in recent years. Thanks to the support of government, a three-tier system of obstetric aid has been introduced and the system of perinatal centers has been developed. Historic lows have been achieved in maternal (7.3 per 100,000 live births) and infant mortality (5.5 per 1,000 live births). The setting into operation of federal specialized centers, including perinatal centers, has significantly increased the availability of high-tech medical care to the population.

The commencement address was presented by the Hero of Labor of the Russian Federation, RAS academician G. M. Savelyeva; the speech was devoted to the author’s view on the problems of obstetrics and perinatology. The report presented the main achievements in modern obstetrics and perinatology. The speaker mentioned the peculiarities of pre-eclampsia in hospitalized patients, as well as the ways of reducing maternal mortality in this pathology. The problems in the specialty area highlighted by the academician included preterm delivery, its causes and consequences; maternal mortality and the analysis of its structure; pregravid preparation, including high-risk patients; treatment policy under medical emergency in obstetrics, including postpartum hemorrhage, placenta increta.

The opening ceremony of the Forum and the Congress featured the following speakers: M. A. Murashko, head of Roszdravnadzor (Federal Service on Surveillance in Healthcare and Social Development of the Russian Federation); Sarah Johnson, President of the International Society of Ultrasound in Obstetrics and Gynecology (ISUOG); Gian Carlo Di Renzo (Italy), Honorary Secretary of FIGO.

The participants were greeted with welcoming telegrams from the Minister of Health of the Russian Federation V. I. Skvortsova, Minister of Industry and Trade of the Russian Federation D. V. Manturov.

The Head of the Forum, RAS academician, G. T. Sukhikh, presented the program of the Forum events. Then he made a report in which he noted active scientific and educational efforts of the RSOG aimed at the implementation of the principles of continuous medical education in various regions of the country, including distance education schools. He noted the high scientific potential of V.I. Kulakov National Medical Research Centre of Obstetrics, Gynecology and Perinatology in basic and applied research and its educational activities. He analysed the evolution of new technologies in obstetrics, gynecology and perinatology The Head of the Forum focused on the role of intestinal dysbiosis and metabolic diseases, as well as the established causal relationship between microbiota and carcinogenesis. He highlighted the role of uterine microbiota in the maintenance of normal endometrium physiology; he also spoke about the transfer of microorganisms from mother to child resulting in the formation of the intestinal microbiota of the newborn. In his report the Head of the Forum mentioned such issues as the impact of the mode of deliveryon the microbiota of the newborn, the cellular nature of the epigenetic information in normal cells and in cancer cells, the effect of maternal nutrition on the signal pathways in the placenta and fetal growth.

The Deputy Director of the Department of Child Healthcare and Delivery Service of the Ministry of Health of the Russian Federation, professor O. S. Filippov presented a report on the results of operation of the obstetric aid in 2017. He made an analysis of the main indicators of maternal and infant mortality in the Russian Federation, as well as reproductive losses. The positive dynamics of abortion prevention was noted, and abortion losses were compared among regions. Pursuant to Presidential Decree No. 204 dated 07.05.2018, the participants were presented with the global strategy for the protection of health of women, children and adolescents until 2030, the elimination of preventable mortality of mothers, newborns, children and adolescents. He also listed the tasks of health authorities aimed at the support of sustainable population growth in the Russian Federation and life expectancy increase.

The Head of Roszdravnadzor, professor M. A. Murashko, emphasized the key measures on the providing and maintaining the patient’s safety, high quality of medical care, implementation of standardization, IT penetration and communication processes in health care and pharmaceutical support.

The opening of the Forum and Congress featured a ceremony of awarding the Diplomas of Merit of the Minister of Health of the Russian Federation and the badges “Excellent Worker of Public Health of the Russian Federation” to the best public health care practitioners, researchers and health care organizers.

The Forum included National Research and Education Echographic Congress “Ultrasound Diagnosis in Obstetrics, Gynecology and Perinatology: from Basic Principles to Innovative Approaches” with the ISUOG support; the VIII research and practice conference “Miscarriage: Social Problem, Medical Solutions”; five pre-courses on emergency obstetrics, aesthetic gynecology, infectious and inflammatory diseases in neonatology, diseases of the uterine cervix and vulva, ultrasound diagnosis of chest and abdominal anomalies of the fetus; two post-courses in neonatal cardiology, magnetic resonance imaging in obstetrics, gynecology and neonatology; the IV Conference of the League of Midwives of Russia; the meeting of chief obstetricians and gynecologists of the Russian Federation; a contest of research projects of young scientists.

The organizational meeting of the VI Congress of Obstetricians and Gynecologists of Russia included the report of the RSOG President, academician V. N. Serov, on the activities of the Society over the past five years. It was followed by the discussion of the report, the election of the RSOG Board and the President of the Society. The new Board and the Presidium of the RSOG Board were presented at the Congress. RAS academician V. N. Serov was elected the President of the Society again.

The research part of the Congress and Forum was devoted to the topical issues of obstetrics, gynecology and neonatology: treatment policy under medical emergency in obstetrics, premature birth, miscarriage, prevention and treatment of pre-eclampsia, the outstanding issues of gynecologic oncology, mammary gland pathology, fundamental and clinical issues of endometriosis and endometrial hyperplasia, infectious and inflammatory diseases of the female reproductive system, new technologies in operative gynecology, new technologies in reproductive medicine, extragenital diseases in the practice of obstetricians and gynecologists, modern possibilities of treatment of pelvic floor dysfunction, contraception, pediatric gynecology, neonatology, medical rehabilitation in modern gynecology, andrology. The participants discussed the prospects of the development of obstetric and gynecological care in the country, including the improvement of postgraduate education of doctors. There were 613 reports at more than 100 plenary sessions, section meetings, topical seminars, symposia, round tables, schools and master classes.

The plenary session “Obstetric Hemorrhages” (chairpersons: RAS academician M. A. Kurtser, prof. R. G. Shmakov, prof. A. V. Pyregov) featured an active discussion of clinical guidelines for the management of major obstetric hemorrhages, anaesthetic aspects of massive hemorrhage treatment, organ sparing surgery for uterine rupture.

Speakers who presented their reports at the plenary session “Endometriosis: Fundamental and Clinical Data” (chairpersons: RAS academician L. V. Adamyan, RAS corresponding member I. Yu. Kogan, prof. V. F. Bezhenar, V. E. Frankevich, M. Yu. Bobrov), analyzed the current trends in pathogenesis, diagnosis and modern treatment of endometriosis.

Participants of the plenary session “Prospects for the Development of Reproductive Medicine” (chairpersons: RAS academician G. T. Sukhikh, RAS academician L. V. Adamyan, RAS academician V. N. Serov, academician L. A. Ashrafyan, RAS academician G. M. Savelyeva; G.C. Di Renzo, A. Bashiri, N. Zhong) discussed the current trends in the development of reproductive medicine, the issues of reducing reproductive losses in the early and late stages of gestation, new international guidelines on recurrent miscarriage. Of particular interest was the presentation of the Russian version of the Maternal and Child Health Handbook (Boshi-Techo, Japan).

At the plenary session “Placenta Increta as a Challenge for Perinatal Obstetrics” (chairpersons: RAS academician M. A. Kurtser, prof. R. G. Shmakov, prof. N. V. Protopopova, prof. L. D. Belotserkovtseva, prof. I. A. Salov, prof. V. B. Tskhay, prof. G. B. Malgina, prof. I. O. Bushtyreva) a multi-faceted discussion of diagnostic and prognostic criteria for the evaluation of placenta increta was held, including the uterine scar after cesarean section, the effectiveness of endovascular methods of uterine artery embolization, pregnancy management and tactics of delivery in cases of placenta increta.

Two plenary sessions chaired by RAS academician G. T. Sukhikh, prof. D. N. Degtyarev, prof. I. V. Ivanov, prof. N. V. Dolgushina, M. P. Shuvalova were devoted to the role of V. I. Kulakov National Medical Research Centre of Obstetrics, Gynaecology and Perinatology in the development of innovations and achievement of high quality of medical care. At the first session, the participants’ attention was drawn to the issue of improvement of the quality and safety of perinatal care on the basis of practical recommendations of Federal Service for Surveillance in Healthcare (Roszdravnadzor), the introduction of telemedicine technologies in obstetrics, gynecology and neonatology. The issues discussed at the session included the improvement of the quality of training, the role of simulation technologies in the reinforcement of the human resources potential of perinatal facilities. At the second session, the prospects of cooperation with the subjects of the Russian Federation were outlined. Modern technologies in obstetrics, oncology, as well as issues involved in the formation and development of neonatal service, were discussed. A special topic raised by the participants was the professional standard of an obstetrician-gynecologist.

A wide range of topical issues of pathological obstetrics was examined at the VIII Research and Practice Conference on Miscarriage chaired by prof. N. K. Tetruashvili. The participants discussed the role of genetic and immunological factors in early miscarriages, surgical correction of complications caused by cervical incompetence. Particular attention was paid to the importance of thrombophilic status in recurrent miscarriage. The recommendations of the European Society of Human Reproduction and Embryology in the field of examination and management of couples with recurrent miscarriage were presented to the participants of the conference. Experience in treating this category of patients in Russia was summarized. The role of septic infection in reproductive medicine was mentioned, and the prognosis in that aspect was made; the participants discussed the prevention and correction of venous thromboembolic complications, rhesus sensitization and hemolytic disease of the fetus. A significant place in the program was given to the discussion of extragenital diseases in the practice of obstetricians and gynecologists. The national recommendations for the diagnostis and treatment of cardiovascular diseases in pregnancy were presented for revising and updating. At the breakout session, special attention was paid to the discussion of the national experience and international data on early miscarriage (chairpersons: Asher Bashiri, RAS academician V. N. Serov, prof. N. K. Tetruashvili).

A number of seminars and breakout sessions were devoted to the possibilities and prospects of fetal diagnostis. In particular, the breakout session “Fetal Ggrowth: Evaluation Standards and Clinical Practice” (chairpersons: (Stephen Kennedy, Jose Villar, Aris Papageorghio, prof. A. I. Gus, prof. Z. S. Khodzhaeva, prof. I. I. Ryumina, prof. O. R. Baev) discussed INTERGROWTH-21st international fetal growth standards, which are the parameters of fetal growth and child’s growth from conception to the age of 5. Breakout session “The Fetus as a Patient” (chairpersons: RAS academician M. A. Kurtser, prof. N. V. Bashmakova, prof. R. G. Shmakov, prof. N. K. Tetruashvili, prof. E. L. Bokeria, prof. I. E. Zazerskaya, K. V. Kostyukov, K. A. Gladkova) featured a discussion on the possibility of fetal surgery in cases of amniotic cord syndrome, complicated monochorionic twins, intrauterine surgical correction of spina bifida. Successful cases of treatment of fetal non-immune hydrops were presented, as well as the experimental method of sealing iatrogenic ruptures of membranes.

Under the chairmanship of RAS academician E. K. Ailamazyan and RAScorresponding member I. Yu. Kogan, a breakout session on great obstetrical syndromes was held. The panel discussion on delivery tactics and its management in modern obstetrics was of great importance (chairpersons: RAS academician M. A. Kurtser, RAS corresponding member V. E. Radzinsky, prof. O. R. Baev, prof. A. A. Mikhailov, prof. G. A. Penzhoyan, prof. I. A. Salov, A. V. Nikolayeva).

At the breakout session and presentation of the monograph “Life-Threatening Conditions in Obstetrics and Perinatology” (chairpersons: RAS academician Strizhakov, RAS corresponding member A. D. Makatsariya), modern approaches of prenatal consultation of patients with habitual miscarriage were substantiated, and the participants discussed the possibilities of prevention of reproductive losses in high-risk pregnant women and the critical state of the fetus in full-term pregnancy.

A traditional topic of the discussion was the issue of pre-eclampsia. At the breakout session (chairpersons: RAS academician I. S. Sidorova, I. A. Panova, prof. R. I. Shalina, V. S. Chulkov), a new concept of pre-eclampsia development was substantiated, and the participants discussed molecular genetic predictors of preeclampsia. A discussion was held based on the Russian and the international recommendations “Hypertension in Pregnant Women”.

Much attention at the forum was paid to the discussion of innovative approaches to the prevention and treatment of benign and malignant neoplasms of female reproductive system. At breakout session “Cancer and Pregnancy” (chairpersons: academician L. A. Ashrafyan, A. A. Parakonnaya, prof. R. G. Shmakov), clinical recommendations on issue “Cancer and Pregnancy” were presented by Frederic Amant (Belgium), and a report was made on the 20-year experience of V. I. Kulakov National Medical Research Centre of Obstetrics, Gynaecology and Perinatology in the tactics of managing pregnant women with cancer.

A special breakout session was devoted to the problem of endometriosis and featured a discussion of a wide range of issues: from molecular/biological mechanisms of disease pathogenesis and new markers to new therapy possibilities (chairpersons: RAS academician L. V. Adamyan, RAS corresponding member I. Yu. Kogan, prof. V. F. Bezhenar, V. E. Frankevich, M. Yu. Bobrov).

Breakout sessions were devoted to the achievements of gynecology and confirmed the preference for organ-sparing approaches in surgical gynecology, the importance and accuracy of the diagnosis of reproductive system pathology.

Round table discussion on early reproductive age (chairperson: prof. E. V. Uvarova) demonstrated new achievements in the study of pathogenesis and the development of modern approaches to the treatment of the most complex gynecological diseases of adolescence and the importance of active prevention of abortion.

Chronic inflammatory processes of pelvic organs in obstetrics and gynecology were one of the subjects of discussion at a number of seminars and round tables. At seminar “Infectious and Inflammatory Diseases in Obstetric Practice” (chairpersons: prof. N. V. Artymuk, V. L. Tiutiunnik, A. V. Nikolaeva, S. M. Voevodin, prof. L. Yu. Karakhalis, S. V. Novikova), the participants discussed the diagnosis and prevention of intrauterine urinary tract infection in pregnant women, postpartum infectious and inflammatory diseases, and prevention of chronic endometritis. At the round table discussion “Infectious and Inflammatory Diseases of the Female Reproductive System: New Treatment Options” (chairpersons: RAS academician V. N. Serov, prof. I. I. Baranov), reports were presented on preconception preparation of patients with recurrent vulvovaginal candidiasis.The arguments were provided in favour of the efficacy of aminodihydrophthalasindione in complex treatment of chronic endometritis in teenage girls with recurrent uterine bleeding; the use of immunomodulatory therapy in complex treatment of pelvic inflammatory diseases (PID) was discussed. At seminar “Infectious and Inflammatory Diseases of Genitals in Women of Reproductive Age” (chairpersons: prof. I. I. Baranov, prof. T. Yu. Pestrikova, prof. G. B. Dikke), the report was presented on the results of a multi-center open randomized comparative study on PID treatment. The report on the XIV International Symposium on Human Chlamydial Infections (Netherlands, 2018) was made.The participants also discussed topical issues of chronic endometritis treatment.

The issues related to the new opportunities of overcoming infertility in the XXI century were discussed at the breakout session on reproductive medicine (chairpersons: RAS academician L. V. Adamyan, RAS corresponding member I. Yu. Kogan, prof. V. F. Bezhenar, prof. I. A. Salov, prof. I. F. Fatkullin, A. V. Kozachenko, S. A. Martynov, D. Yu. Trofimov).

The use of contraceptives, rational hormone therapy of endometrial hyperplastic processes, pelvic floor dysfunction, health and quality of life of women in different ages were the topics of breakout sessions, seminars and round table discussions.

A group of breakout sessions and seminars was devoted to topical problems of neonatology and pediatrics; these involved a wide range of issues on the effectiveness of clinical audit, modern specifics of resuscitation and intensive care. The issues of infectious and inflammatory diseases in newborns, their feeding, and cell therapy of severe pathology in infants, as well as the role of obstetric tactics in determining neonatal outcomes in late delivery were also discussed. Special attention was paid to skin diseases of newborns. The draft Protocol on RDS and the pilot project “Study of distribution and intensity of circulation of strains of pathogens (including resistant ones) of infectious diseases among pregnant women, puerperas and newborns in the regions of the Russian Federation” were highlighted. Neonatologists noted the unsolved problems related to a number of drugs widely used abroad (vitamin K, liquid phenobarbital, liquid phosphorus, etc.)and not introduced into practice in our country.

At the National Research and Academic Echographic Congress “Ultrasound Diagnostics in Obstetrics, Gynecology and Perinatology: from Basic Principles to Innovative Approaches (supported by ISUOG)” (chairpersons: RAS academician G. T. Sukhikh, RAS academician S. K. Ternovoy, Sarah Jonson, prof. V. V. Mitkov, prof. S. M. Voevodin, prof. A. N. Sencha, prof. V. N. Demidov, prof. A. I. Gus), the participants discussed the most topical issues of the use of ultrasound diagnostics in obstetrics, gynecology and neonatology; they outlined the modern principles of diagnostics and described the planned prospects for further development of ultrasound technologies.

Within the framework of the Forum, Schools were held on such issues as endoscopic surgery in the practice of gynecology, andrology, hemostasis in perinatal medicine, HPV-caused cervical pathology, and hormonal contraception.

A significant place in the scientific program was devoted to the prevention and treatment of HPV-associated diseases. At seminar “Papillomavirus Infection and Reproductive Health” (chairpersons: prof. L. D. Belotserkovtseva, prof. O. G. Pekarev, Prof. N. V. Zarochentseva, prof. T. Yu. Pestrikova, N. M. Nazarova), reports were presented on the experience of the regions in primary prevention and treatment of HPV-associated diseases, and the role of the virus in the development of cervical and anal neoplasia was discussed.

The range of problems discussed at the Forum and the Congress was extremely broad, and the scientific program covered the professional interests of doctors of all special areas. It was mentioned that, despite the results achieved, a number of issues remain unresolved. Attention is drawn to the prevalence of extragenital pathology, which occupies a leading place in maternal morbidity and mortality. The deterioration of reproductive health of women, especially younger girls and teenage girls, as well as the growing numbers of malignant neoplasms of the female reproductive system are the causes for concern. It is high time to improve pregravid preparation and prenatal diagnostics. The solutions to the issues of standardization of obstetric and gynecological care, the introduction of modern technologies, and the enhancement of postgraduate education of obstetricians and gynecologists with due account for the need to implement high-tech medical care, are being provided slowly.

Following the discussion, the participants of the XIX All-Russian Research Forum Mother and Child and the VI Congress of Obstetricians and Gynecologists of Russia consider it necessary to recommend the following:

Head obstetricians and gynaecologists of the health authorities of constituent entities of the Russian Federation, heads of departments of Obstetrics and Gynecology of medical universities, and chairpersons of the regional offices of RSOG are instructed as follows:

  • it is necessary to intensify the activities in the field of joint training programs in modern management technologies aimed at improving the efficiency of resource use and the enhancement of the quality and safety of medical care;
  • to strengthen analytical and research work to improve the mechanisms of medical organizations management, as well as to determine the necessary level of competence of the administrative and managerial personnel in the sector;
  • to take measures for rational use of regional perinatal centers and follow the recommendations for the delivery in cases of high-risk pregnant women in obstetric institutions of level III;
  • one of the priority directions in the work on reducing maternal mortality should be the reduction of the share of caesarean sections. The possibilities to reduce the frequency of caesarean sections are based on the following reserves: strict regulation of relative indications for cesarean section; the use of modern methods of preparation of women with uterine scar to natural childbirth;
  • to expand the possibilities of surgical correction of cervical incompetence complications, to apply fetal surgery more frequently, especially in cases of multiple monochorionic pregnancy;
  • to expand the scientific and practical interdisciplinary approach in solving the problems of gynecologic oncology and onco-obstetrics;
  • to develop and implement clinical guidelines and treatment protocols;
  • to take into account that significant changes have taken place in the structure of the causes of maternal mortality: in particular, severe bleeding due to chorion presentation and ingrowth, hemostasiological hemmorhage in cases of preeclampsia, dead fetus, multigenic thrombophilia;
  • to increase the work on the implementation of non-invasive prenatal diagnostic methods with the use of maternal blood;
  • to increase the efforts towards the prevention of abortion;
  • to adhere to the recommendations of the national guidelines on obstetrics, neonatology and gynecology in the organization of the diagnostic and treatment process.

2. To ask the Ministry of Healthcare of the Russian Federation:

  • to arrange a system of relationships between university subdepartments and healthcare institutions;
  • to provide legislative basis to ensure the position of clinical disciplines professors as doctors and not just educators; to allow the faculty of medical sub-departments to conduct medical work in the clinics of the subdepartments;
  • to request the acceleration of clinical trials of a number of necessary medications which are widely used in modern international neonatal practice.

To the Bureau of the RSOG Board:

  • to strengthen the work on the improvement and professional development of doctors in order to reduce maternal and perinatal morbidity and mortality;
  • continue preparing and updating evidence-based clinical guidelines and protocols; to reissue the national guidelines on obstetrics and gynecology, which will make it possible to improve the obstetric and gynecological practice;
  • to discuss at the Presidium of the RSOG Board the issue of revising the age of fetal viability from 22 to 24–25 weeks of pregnancy, taking into account national and foreign experience; and issue of the organization of legal assistance to doctors by the professional community;

The Board of RSOG has to pay special attention to the organizational issues in the work of the society aimed at the solution of the tasks of continuous education, the need for public participation in determining the category of doctors, certification of obstetric and gynecological institutions, creation of new conditions for the work of regional societies;

  • to continue holding annual forums and plenary sessions of the society, as well as research and educational schools of RSOG in different regions of the country;
  • continue to participate in the work of Obstetrics and gynecology journal. To recommend to the heads of regional societies to provide subscription to Akusherstvo i Ginekologia (Obstetrics and gynecology) journal in its hard-copy or electronic versions.
